Health Requirements for Students

As our community slowly emerges from COVID-19, Fall 2020 classes will be taught both online and face-to-face. Students will need to update the following health requirements before August 7, 2020 (The last day to register for Fall classes)

Please follow these health requirement guidelines if you intend to register for classes for the Fall 2020 semester:

1. TB skin test (visit your clinic for testing);  if +Reactor to TB, send an email message to the Health Services Center (HSC) for further guidance;

2. MMR (Measles/Mumps/Rubella) - 2 doses required (this vaccine is given in 2 doses during childhood years, if a record is available showing these doses, this vaccine is not needed for the rest of a person's lifetime); If no record is available, visit a clinic to have both doses administered;

3. TD/TDAP/Booster - this vaccine is good for ten (10) years; and

4. Emergency and Health Information - This form can be found at the bottom of this page.

Students will be required to submit ALL health requirements for Fall, 2020 by or before August 7, 2020 (1 week prior to the start of the term).
